When returning to our office in the era of COVID-19, there will be changes in office operation and procedures to keep you and our staff members safe. Our office is adhering to CDC and AOA guidelines for re-opening the practice.
Our office staff will be provided with PPE, there will be breath shields at the front desk, for the doctor and opticians, and at various equipment points.
Equipment will be cleaned & sanitized between patients, initially we will have reduced patient flow.
Patients will be screened when making appointments as to medical history and potential contact with COVD-19 patients.
If you are a new patient, please download Welcome to Our Office Form, and have it filled out upon arrival.
Upon arriving at office, give us a call at 408-376-0460, a staff member will call you back and let you know when to come in to the office. Temperature will be taken with an IR non-contact thermometer at entrance.
Patients entering office will be required to be wearing a mask that covers mouth and nose. If you do not have a mask, we will supply you with one. Patients wearing gloves should remove them before entering the office. We have hand sanitizer that will be mandatory upon entering the office.
Patients will no longer be allowed to bring food or drink in to the office,
Parents with children coming in for examination: only one parent allowed to accompany child due to social distancing guidelines.
The dispensary area will have reduced seating to help ensure social distancing guidelines are met.
Opticians will bring frames to you for frame selection, frames that have been tried on will be cleaned and disinfected prior to returning to our frame board. Patients will not be allowed to handle frames on their own.
Appointments will be required for eyeglass dispensing, repair, & adjustments, again, due to social distancing guidelines.
